Puppy Kindergarten Registration Form
Fill out the form below completely in order to sign up for the class. Classes may be paid via check or cash brought to your first class day or you may pay via paypal. Paypal payments include an additional 3 dollar convenience fee in the total. Please be aware, this class is ONLY for puppies 5 months of age or younger by session start date. If your puppy will be older than 5 months, please apply to our fundamentals of obedience class.
